Indications/Uses
Type 2 DM in adults when dietary measures, physical exercise & wt loss alone are not sufficient to control blood glucose.
Dosage/Direction for Use
From 30-120 mg once daily at breakfast, depending on response, including elderly or those w/ mild to moderate renal insufficiency. Patients at risk of hypoglycemia Initially 30 mg daily. Combination w/ other antidiabetics May be given in combination w/ biguanides, α-glucosidase inhibitors or insulin.
Administration
Should be taken with food: Swallow whole, do not chew/crush.
Contraindications
Hypersensitivity to sulfonylureas, sulfonamides. Type 1 diabetes, diabetic precoma & coma, diabetic ketoacidosis, severe renal or hepatic insufficiency, co-administration w/ miconazole. Lactation.
Special Precautions
Hypoglycemia may occur w/ all sulfonylureas, in cases of accidental overdose, when calorie or glucose intake is deficient, following prolonged or strenuous exercise & patient w/ severe hepatic or renal impairment. Inform patient of importance of following dietary advice, taking regular exercise & regular monitoring of blood glucose levels. Patients w/ G6PD deficiency. Possible symptoms of hypoglycemia should be taken into account prior to treatment. Pregnancy (change to insulin before pregnancy is attempted or discovered).
Adverse Reactions
Hypoglycemia, abdominal pain, nausea, vomiting, dyspepsia, diarrhea, constipation, changes in haematology generally reversible. Raised hepatic enzymes levels, hepatitis. Cholestatic jaundice (discontinue treatment). Transient visual disturbances at start of treatment, rash, pruritus, urticaria, angioedema, erythema, maculopapular rashes, bullous reactions eg, SJS, TEN, & DRESS. Erythrocytopenia, agranulocytosis, haemolytic anaemia, pancytopenia, allergic vasculitis, hyponatremia, elevated liver enzymes, liver function impairment (cholestasis, jaundice) & hepatitis.
Drug Interactions
Increased hypoglycemic effect w/ miconazole, phenylbutazone, alcohol, other antidiabetic agents, β-blockers, fluconazole, ACE inhibitors, H2-receptor antagonists, MAOIs, sulfonamides, clarithromycin, NSAIDs. Blood glucose levels may be increased by danazol, chlorpromazine, glucocorticoids & ritodrine, salbutamol, terbutaline. St. John's wort (Hypericum perforatum) prep. Risk of dysglycaemia w/ fluoroquinolones. Potentiation of anticoagulants therapy.
